Our mission
Readmissions often begin with a broken handoff — not a lack of effort.
Hospitals, physicians, nurses, post-acute teams, and families are all working hard. What's missing is the connective tissue between them in the days after discharge. Loopcare exists to be that connective tissue — a coordinated workflow with a human on the other end, making sure every loop actually closes.
What we believe

What we don't do
We don't practice medicine, replace clinical judgment, or make treatment decisions. We coordinate the plan the care team has already set — and make sure it's followed through.
